Clinician-reviewed documentation

VitalRep™

VitalRep™ helps a clinician turn selected consultation information into a structured, editable note that remains under their review and final approval.

Clinician notes are selected for a draft; no clinician-approved final note is shown.

Built for the care workflow

For the clinician responsible for the appointment and its clinical record.

Capture and transcription require their own consent and provider acknowledgement. A drafted note does not prove that live transcription occurred.

An editable SOAP draft

The draft organises the selected source into sections for the clinician to inspect and correct before final approval.

Editable VitalRep SOAP draft marked Draft, with Subjective, Objective, Assessment and Plan sections.

How it works

Choose the consultation source

Use an available appointment transcript, clinician notes, or both. If no transcript exists, the clinician can work from their own notes.

Prepare and edit a draft

Fola-assisted drafting organises the selected text into SOAP sections. A separately labelled structured fallback is available. The clinician reviews and edits every section.

Save, then finalise

The clinician can save an editable draft and later finalise the note. The final record is protected by a separate amendment path.
